What is Liposuction?

Liposuction is a procedure that surgically removes fat from problem areas. It is a great option for addressing pockets of fat on your:

When diet and exercise aren’t enough to help you reach your body contouring goals, liposuction can help. Generally the body gains and loses weight relatively evenly over the whole body, but there are certain areas that tend to hold fat longer than others and it can be difficult to remove these problem areas.

The Liposuction Procedure?

Typically, Dr. Nicholas Lahar, Beverly Hills Plastic Surgeon performs liposuction under general anesthesia. A salt water solution containing numbing medication and a medication to reduce bleeding is gently injected in the areas to be treated. This fluid helps to buffer the tissue and skin from injury and prepares the fat for removal.

This technique makes the procedure safer and more comfortable. Introducing fluid also helps break apart fat deposits, easing removal.

Once the fat has been dislodged from the surrounding tissues, I then use a thin tube called a cannula to remove unwanted fat. This technique is more gentle than just pulling the fat out immediately and reduces bleeding and helps with recovery.  A medical vacuum device creates the suction needed to remove fat from your body.

The entire procedure only requires a few small incisions and closure techniques, and incision placement ensures minimal visible incision lines once healing is complete.

Depending on the volume of fat to be removed, liposuction usually takes between one and five hours, depending on the area of your treatment and the amount of fat being removed. You will experience swelling and bruising in the days after your procedure, but the major swelling will fade within a few weeks.

At that point, you can begin to enjoy a sleeker, more refined contouring in the treatment area. You will want to maintain compression of the area to control swelling as you heal.

Liposuction Recovery Time Month by Month

The total time to healing can range from 4 to 6 months and sometimes longer for patients who have a revision or second round of Lipo.

In these patients, there is more of a liposuction scar and the recovery can be a little more difficult depending on what the tissues are like.

What is VASER liposuction?

VASER is a well-established technique and technology to help break up fat into fine liquid particles. It stands for Vibration Amplification of Sound Energy at Resonance. These fancy words actually do say a lot about what the technology is.

In classic liposuction, fluid is injected into the fat to clamp down blood vessels within the fat so that there’s less bleeding. Once the fluid has taken effect, the fat is mechanically broken up and then suctioned out of the body with a thin hollow tube called a cannula. VASER liposuction adds a step prior to suction of the fat. A solid thin tube is placed into the fat to be removed.

This solid probe vibrates at a very high frequency and causes small bubbles to form in the fluid within the fat. These bubbles knock fat off of the surrounding tissues without scraping them out like in liposuction. What is left is the intact surrounding tissue and liquefied fat that is suctioned out more easily and with less trauma. For larger areas of fat removal I do like to use this technology to reduce soreness, make for easier fat removal, and to help in the recovery of the patient.

All procedures can be combined together, this is, of course, within reason. Liposuction FAQs

Liposuction, particularly smaller areas, can be performed under local anesthesia. The fluid that is injected into the fat is adjusted slightly to increase the numbing medication to make for a more comfortable experience.  You may be given a light medication prior to the procedure in order to relax you. You will need a ride home if you take the medication.

While many of us desire maximum results, safety is of utmost importance in surgery, including liposuction surgery. While small amounts of liposuction such as removal of fat in one area is extremely safe, larger volumes of liposuction such as over 5 L has an increased risk of having complications. These include fluid collection underneath the skin, contour irregularities, and low blood levels.  For total body liposuction cases, it is very important to be screened for low blood count and your ability to tolerate surgery during your consultation. Depending on your baseline health and other parameters, it may even be necessary to split your surgery into two stages or more for safety reasons. The American Society of Plastic Surgeons recommends any large volume fat removal (>5 Liters) be performed in an accredited facility or hospital.  All of my surgeries are performed in an accredited facility.

Even in small areas where “maximum removal” is desired, this is rarely a good idea. It is important to leave a small layer of fat under the skin, otherwise the skin scars to the underlying muscle and looks very distorted and unappealing.  Liposuction is at its best when different amounts of fat are removed or left in place to create a body with aesthetic curves, hills, and valleys.

Because the number of fat cells in adults is fixed, liposuction will remove fat from treated areas permanently. However, any weight gain after will still make the existing fat cells larger with fat if nutrition and exercise are not implemented after surgery. This can include fat that sits around the organs internally as well.  If this internal fat accumulates enough, it can contribute to a large, protruding abdomen despite having little fat just under the skin.

The only way to treat this excess internal fat is through good diet and exercise.

Fat that is removed from your body can definitely be placed (“grafted”) into other areas of your body that may be lacking in size or contour. Fat grafting to the buttocks and breasts are two increasingly popular procedures that patients are using to help them reach their goals.

This is another technology that I will use in certain cases to help with skin contraction after liposuction. After removal of the fat layer underneath the skin, there is an empty space that will remain under the skin, but above the muscle.

This means that the skin is hanging relatively loose on the abdominal muscles below. In order to improve skin shrinkage to the tissues below, Renuvion can shorten some of the fibers that the fat used to live in before being removed. This helps to pull the skin towards the underlying muscle and tighten the skin.

The technology is well controlled and preferentially chooses the fibers that are more hydrated and longer. The skin tightening is noticeable in the operating room and seems to increase over time.

The technology is not a miracle worker and if you have truly loose skin that is hanging, you will require a tummy tuck or skin excision procedure to tighten the tissues over your frame.

That being said, if you are a borderline case, it may be worth it to attempt to tighten the skin after liposuction to save you a longer incision. If down the line you are unhappy with the looseness in your skin we can do the skin excision procedure.
