What are the bands under my chin?
Those vertical lines or cords that become visible in the neck are often caused by a structure called the platysma muscle. The platysma is a thin, flat sheet of muscle that runs from the collarbone up to the jawline, covering the front of the neck.
As we age or lose fat in the neck area (such as through liposuction), the platysma can become visible—especially if it is loose, separated, or overly strong. These lines are often referred to as platysmal bands, and they can appear more noticeable after fat has been removed from over the muscle.
A StatPearls clinical reference published on NCBI notes that avoiding over-resection of fat during neck contouring is important. Aggressive fat removal can lead to skeletonization of the neck and unmask the underlying platysma in an aesthetically unfavorable manner. It shows the importance of a conservative technique for a natural result.
Why does this happen after Chin Liposuction?
Chin liposuction is designed to remove excess fat beneath the chin to reveal a more defined jawline and neckline. However, if done too aggressively—or if the underlying anatomy isn’t ideal—removing too much fat can unmask a problem that was already there: platysmal laxity.
Here are the main reasons bands might appear post-liposuction:
- Loose platysma muscle: When the fat is removed, any existing weakness or drooping in the muscle becomes more visible.
- Platysma “bowstringing”: In some patients, a strong, tight platysma can pull in a way that causes visible vertical cords or bands, particularly with movement or facial expressions.
- Aging anatomy: With age, the platysma can weaken and separate, causing it to hang more visibly, especially when the supportive fat layer is thinned.
A retrospective analysis of 301 patients treated with submental liposuction was published in PubMed. It found that submental obesity and a specific anatomic pattern of platysmal nondecussation (where the platysmal fibers do not cross midline) were significant correlates of postoperative anterior platysmal banding.
This finding established that careful preoperative anatomical assessment is key to identifying patients at higher risk for this outcome before surgery.
This is why chin liposuction must be performed carefully and judiciously. While fat removal can be incredibly effective for jawline definition, overly aggressive suctioning or poor patient selection can unintentionally highlight underlying muscle issues.

Can this be prevented?
Yes—and this is where surgical expertise really matters.

Can platysmal bands be treated after Liposuction?
Absolutely. If you’re noticing bands after your chin liposuction, there are solutions:
- Botox injections can help soften the appearance of mild platysmal bands by relaxing the muscle.
- For more significant or persistent bands, a neck lift with platysmaplasty can surgically tighten and repair the muscle for smoother results.
- In select cases, revision surgery may be considered to balance the fat removal and address underlying muscle visibility.
A multicenter retrospective study of 391 patients was published in the Aesthetic Surgery Journal (PubMed). It found that percutaneous suture suspension platysmaplasty is a safe and viable option for improving neck contour in patients with prominent anterior platysmal bands.
The procedure had a 1% infection rate and a 4.6% rate of recurrent bands. Patients also reported high satisfaction at follow-up. The study also confirmed that the technique can be combined with submental liposuction for patients who need both fat reduction and muscle correction.
The combination approach (liposuction plus platysmaplasty) is often what we recommend for patients with moderate to significant platysmal laxity. Doing both in a single session is more efficient, more cost-effective, and gives a more cohesive result than treating them separately.

While chin liposuction is often seen as a quick, minimally invasive fix for a double chin, it’s not a “one-size-fits-all” solution. Removing too much fat—especially in patients with muscle laxity—can create new aesthetic concerns like visible bands or an over-sculpted appearance.
